Make Your First Visit to a Periodontist Beneficial

The first visit to the periodontist is normally a consultation for most of our patients. This consultation involves gathering information from you, reviewing the records sent by your dentist, examining your mouth, and discussing treatment and therapy options. A typical first visit at Integrated Periodontics lasts from 30 to 75 minutes.

Before this appointment, you will be asked to complete some paperwork. You'll start by filling out forms providing us your general medical and dental history, as well as information about your dental insurance coverage. You will be asked to read and acknowledge receipt of our privacy policies regarding your medical records and personal information and also our “Disclosures required by the Fair Credit Billing Act."

You must read and sign a “Consent for use and disclosure of health information” form before we can begin to treat you. You may download and print these forms provided below and then complete them at your convenience and bring them with you to your first visit.

We will also mail these forms to you at your request. If you prefer to complete these forms on the day of your appointment, please plan to arrive 15 minutes early, to allow time to finish the paperwork before your scheduled appointment. Your First Periodontal Consultation Process

The periodontist will review your medical history and dental records, and you will receive a complete oral examination. Our patients are generally referred to our office by their dentists for a specific reason, but a complete exam by the periodontist is necessary before any course of treatment can be recommended. After this examination, the periodontist will discuss with you the nature and extent of any periodontic issues, and will recommend treatment options.

Basic Foundation for Your First Visit

  • Complete paperwork, if possible
  • Stay on your regular course of medications
  • Brush and floss as recommended by your dentist
  • If you take an antibiotic for dental cleanings, please premedicate before your first visit
  • ​Bring your dental insurance information
  • Carry a list of your current medications, if necessary
  • Bring any patient records, x-rays, or other paperwork your dentist has provided
